After gastric bypass surgery he will have to be very strict with his diet eating anything bad even pasta will give him severe stomach pain. You can't do it unless you got the will power.

The only thing to do will be to send him to the miyiga herding camels let him stay there for over a year, in the baadiye he will be way from all the junk food so less temptation, he will eat like our ancestors proteins and dairy and will burn that off on the hard life of baadiye.

Surgery won't be a solution, he'll just gain all the weight back. He needs to change his habits first. Even if he sheds 200 pounds naturally he'll have so much loose skin that he'll still need a surgery regardless.

So as far as I see it, surgery is gonna be inevitable so take the stomach surgery, but only after you can make sure that he won't put on weight again. 407 pounds is morbidly obese, he's only 19 as well, if he keeps this up I'll see him on "My 600 pound life" on TLC
